The II International Menopause Expert Forum was held on November 20, 2020. working group included leading specialists in gynecological endocrinology from Armenia, Belarus, Georgia, Kazakhstan, Kyrgyzstan, Uzbekistan and Ukraine. experts addressed issues related to the early onset of menopausal hormone treatment (MHT), safety profile MHT breast, developed algorithms for managing unscheduled bleeding women taking MHT.Currently, there is also convincing evidence comprehensive benefits timely initiation – during “window opportunity” (in under age 60 or earlier than 10 years after menopause).Personalized approach each patient a priority when choosing tactics symptoms. lower dose estrogen preferred at beginning treatment, cyclic combined regimen perimenopause continuous postmenopause. requires periodic dosage adjustments depending stage reproductive aging, age, efficacy tolerability. incidence up 77% used 40% with regimen. As result, 50% stop MHT, so proper management critical. key clinicians need address are rule-out endometrial cancer establish cause hemorrhage; can be multiple causes but they often benign. should rational does not require discontinuation if no pathology significant risk factors.Breast associated becomes cornerstone prescribing medication. Hormonal therapy gestogenic component makes greatest contribution breast cancer. Studies have shown that progesterone dydrogesterone an increased cancer, synthetic progestogens associated. minimization activities include history, evaluation status, informing patients right components.
